How to
Register with the Surgery
We welcome new patients to our Practice. Please bring along your Medical Card to register. If you cannot find it we have forms you can fill in. The registration process is simple,
just two forms, one gives us all the administration details we need
to move you from your existing surgery and the second gives us
essential clinical information so that we can start to take care of
you before your notes arrive - these can take several weeks. We also
need one form of ID.
If you are not an existing NHS patient we have to verify that you
are entitled to free NHS treatment. This will require proof that you
are entitled to remain in the UK e.g. Visa, entry clearance. Persons
from EU countries, or returning UK Nationals wishing to reside, will
be asked for proof relating to employment or residency, as directed
by the Department of Health.

TEMPORARY RESIDENTS
We are happy to see people staying in the area or visiting relatives
who need a doctor's advice. You can be registered as a Temporary
Resident for up to 3 months. Simply ask at Reception. Proof of
entitlement to free NHS treatment will be asked for if you are not
already an NHS patient.
